USPS Form 1583. What it is and how to fill it out.

Every virtual mailbox customer must sign Form 1583 before we can receive mail on their behalf. Federal law requires it. We send it via DocuSign. It takes about five minutes.

What it is

Form 1583 is a federal authorization form.

The full name is "Application for Delivery of Mail Through Agent." When you receive mail at a virtual mailbox address, a third party — us — is receiving mail on your behalf. Federal law requires your written authorization before any CMRA can do that. Form 1583 is that authorization.

Without a signed Form 1583 on file, we cannot legally accept mail addressed to you. This is a USPS regulation that applies to every CMRA in the country. There are no exceptions.

The form was last revised in June 2024. We send you the current version via DocuSign immediately after signup. You sign it electronically. We submit it to USPS and keep it on file. You never have to deal with it again.

What you need

Two forms of ID. That's it.

Get these ready before you start the DocuSign process. You'll upload photos or scans of both.

ID #1 — Photo ID (required)

Must be government-issued with your photo. One of:

  • U.S. driver's license or state ID card
  • U.S. passport or passport card
  • U.S. military or uniformed service ID
  • U.S. permanent resident card
  • Certificate of Naturalization
  • NEXUS card
  • U.S. university ID card
  • Matricula Consular

ID #2 — Address verification

Must show your current home address. One of:

  • U.S. driver's license (if it shows your current address)
  • Current lease agreement
  • Mortgage or deed of trust
  • Home or vehicle insurance policy
  • Vehicle registration card
  • Voter registration card

The address on this document must match the home address you provide on the form.

One ID can satisfy both requirements if your driver's license shows your current address. It counts as your photo ID and your address verification. You still need to upload it twice — once in each field.

The date your mailbox was opened. We fill this in after we process your form and assign your suite number.

Our Shift Workspaces address and your suite number. We pre-fill this with our facility information.

Type of service

Check one:

  • · Business/Organization Use — check this if the mailbox is for a business. If you check this, you must also complete Section 7 with your business information.
  • · Residential/Personal Use — check this if the mailbox is for personal mail only.

Most Virtual Business Mailbox customers check Business/Organization Use.

Your name and home address

You fill this

Your legal name, phone number, email address, and home address.

Important — your home address

This is your actual residential address, not your business address and not our Denver address. USPS requires this for identity verification. This information is a private USPS record — it does not appear on your business filings or anywhere public. Your business address remains our Denver suite address.

Section 4k asks if you are a court-ordered protected individual. Check Yes only if a court has issued a protective order on your behalf. If Yes, you must attach a copy of the court order.

Authorized individual (optional)

If someone else needs to be able to pick up or manage mail on your behalf — an employee, business partner, or assistant — list them here. They will need to provide their own ID (Sections 10 and 11). Leave blank if not applicable.

Mail transfer address (optional)

If you want all mail automatically forwarded to another address rather than held at our facility, enter that address here. Most customers leave this blank and manage forwarding individually through the online portal.

Business/organization information

Required for business use

Complete this section if you checked Business/Organization Use in Section 3. You will need:

  • Business or organization name
  • Type of business (LLC, corporation, sole proprietor, etc.)
  • Business street address — this can be our Denver address once active, or your existing business address
  • Business phone number
  • Place of registration — the state where your LLC or corporation is registered (Colorado for most customers)

Your ID information

You fill this

Section 8 is your photo ID. Section 9 is your address verification ID. For each you will enter:

  • ·Your name exactly as it appears on the ID
  • ·The ID number
  • ·The issuing entity (e.g. Colorado DMV, U.S. Department of State)
  • ·The expiration date
  • ·The ID type — check the matching box

Sections 10 and 11 are the same fields for your authorized individual, if you listed one in Section 5. Leave blank if Section 5 is blank.

Additional recipients (optional)

If additional people or business names need to receive mail at your suite number, list them here. Each additional recipient requires a separate Form 1583. Contact us if you need to add recipients after initial signup.

Your signature

You sign this

Your electronic signature via DocuSign and the date. By signing you certify that the information on the form is accurate and that you agree to the USPS terms for CMRA mail service.

Our team signs on behalf of Shift Workspaces as the authorized CMRA. We handle this after you return the signed form.

A common concern

Your home address on Form 1583 stays private.

Form 1583 asks for your home address. That's the point of the form — USPS needs to verify who you are before authorizing a third party to receive your mail.

That information goes into a USPS record. It does not go onto your business filings. It does not appear on the Colorado Secretary of State database. It is not shared with marketing companies or data brokers.

Your business address — the one that appears on every public document — is our Denver suite address. That's the whole point of the service.
