After payment, our team sends you a DocuSign link. Form 1583 is a federal USPS document that authorizes us to receive mail on your behalf. It is required by law — no CMRA can accept mail for you without it on file.
The form takes about five minutes to complete. You sign it electronically. We keep it on file permanently. You never have to deal with it again.
What Form 1583 requires:
- ✓Your legal name
- ✓Your home address (kept private — not used as your business address)
- ✓Two forms of ID — a government-issued photo ID is one
- ✓Your signature
Your home address on Form 1583 is a private USPS record. It does not appear on your business filings or anywhere public.
We issue your suite number
